Housing Stabilization is a Home and Community-Based Service (HCBS), and at Nurture Housing we abide by the HCBS requirement.

Our Criteria

Eligibility

In order to qualify for our Housing Stabilization Services, individuals need to fulfill the following eligibility requirements:

- Must be enrolled in Medical Assistance (MA)
- Must be at least 18 years old
- have a disability or disabling condition
- Must require assistance in at least one of the following areas due to a disability: Communication, Mobility, Decision making, Managing challenging behaviors
- Must have experienced housing instability, which includes homelessness, transitioning from an institution, or being at risk homelessness or institutionalization.
